This form was intentionally opened in a separate window, so that you will be able to alternate between the photo gallery and this form.
- Enter all of the Mailing/Billing information. All of the information in this section is required. Your order cannot be processed unless this information is filled out.
- Select your form of payment. Please make sure that if paying by credit card that your credit card information is correct.
- Enter the order information for the photo(s) that you would like to order. For each respective photo, enter the print/order#, the quantity you would like to order and the size of the print.
After you select the print size, the print price and the total will be calculated for you. There is no need to enter any information in the "price" or "total" areas.
Once you have entered all of the information for all of the photos you are ordering, click on the "Calculate Totals" button on the bottom of the form. Your subtotal, shipping charge, tax (if applicable) and grand total will be calculated automatically.
If you need to change or add to any on the information that was previously entered, afterwards click on the "Calculate Totals" button to view the new totals. Please note that clicking on the "Reset this Form" button will clear all of the information
on this form, not just the photo information.
